Incident/Injury Report
Following an incident/injury the presenter and or member fills out and submits the report within 24-48 hours. Attach any photos or written reports. Contact the Secretary for further information

Following an incident/injury to a member the presenter fills in the form. If a member is completing this form add the presenters name.
Include the member's name, address, phone number and email address
Name the class, the place of incident/injury, date and time
Add a description of the incident/injury, action taken and any other relevant information.
Committee contact is
If no report add N/A. If eye witness report add their name and contact details. Attach the report/s.
